GENERAL RESPONSIBILITIES: The Health Home Care Coordinator is responsible for the provision of all aspects of Health Home Care Coordination services for children. The position requires meeting the requirements of the lead Health Home, DOH and WYH to address the behavioral, medical and social risk factors necessary to meet optimal health goals while avoiding hospitalizations.
