POSITION SUMMARY: The Case Manager provides ongoing, documented case management to adults enrolled in Community Health & Recovery, a low-threshold program offering harm reduction, health promotion, care coordination, and recovery support for people who use drugs or are at risk of overdose or drug-related harm.
