Description The Children’s Case Manager/Health Home Coordinator is responsible for providing both care coordination and case management to child/client (and family members, as appropriate) with a focus on person-centered plans of care. The Health Home Coordinator facilitates access to appropriate resources and develops treatment plans specific to the child’s needs.
