Position Summary: The Rapid Rehousing Case Manager provides consistent, individualized, and trauma-informed case management services to youth and young adult households who have transitioned out of homelessness and are living in housing throughout Salt Lake County. The case manager will support the clients in obtaining and maintaining stable housing and addressing current needs as defined by the client.
