The medical license must be free of restriction and no litigation or disciplinary actions may be pending against the incumbent. The incumbent must have Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP), Advanced Life Support in Obstetrics (ALSO), and DEA and State of Idaho Controlled Substance Registration numbers.
